Sports Registration Information

The Winter Sports Season is scheduled to begin on Monday, November 13 2023 for the following JV/Varsity Sports:
Boys & Girls Basketball and Cheerleading
The Winter Sports Season is scheduled to begin on Monday, November 13, 2023 for the following Varsity Sports:
Boys and Girls Bowling, Boys & Girls Ice Hockey, Boys & Girls Indoor Track, Boys Swimming, and Wrestling.
The Winter Sports Season is scheduled to begin on Monday, November 27, 2023 for the following Modified Sports:
Boys & Girls Basketball, Boys & Girls Swimming, and Wrestling.
The Unified Bowling Season is scheduled to begin on Friday, January 24, 2024.
Registration must be completed in FamilyID (www.familyid.com) with all new physical paperwork submitted to the school nurse for Varsity and JV sports by November 8, 2023.
Registration must be completed with all new physical paperwork submitted to the school nurse for all Modified sports by November 21, 2023.
Registration for Unified Bowling will open in FamilyID on Tuesday, January 2, 2024. Registration must be completed with all new physical paperwork submitted to the school nurse for Unified Bowling by Wednesday, January 21, 2024.
The FamilyID registration portal will close at the close of business on the above specified dates. Parents MUST be the ones registering their children and not allowing their children to register themselves on FamilyID.
HEALTH FORMS REQUIRED:
Please make sure you have a valid physical on file in the nurse’s office.
We are no longer using the Sports Candidate Questionnaire form. The information contained on these forms is now embedded in your FamilyID registration. It is imperative that a parent or guardian complete the registration in FamilyID. Reminder, students SHOULD NOT be completing the registration.
Following a positive COVID-19 diagnosis, the District may require that your child be evaluated and cleared by his or her primary care physician (PCP) and to be symptom free before returning to athletics or physical education class. Your child’s PCP will be asked to complete the Assessment/Release for Return to Play form prior to resumption of athletics or physical education. Within this form, your child’s PCP has the option to require an additional 4-Phase Graduated Return-to-Play protocol be completed prior to returning to full activity.
Additionally, if your child needs an inhaler or epi-pen, parents must complete the medication consent form each school year.
All of these forms are located on our website at https://www.starpointcsd.org/domain/81 under the Sports Physical tab. All new physical paperwork must be turned in to either the Athletic Office or the nurse’s offices ASAP. Additionally, paperwork can be faxed to the high school nurse's office at (716) 210-2361, the middle school nurse's office at (716) 210-2231 or by emailing Jane Wilson at jwilson@starpointcsd.org.
No student will be able to try-out without being registered on FamilyID and all of the proper paperwork being completed and submitted to school.

FamilyID REGISTRATION PROCESS:
A parent/guardian can register by clicking on this link: https://www.familyid.com/starpoint-csd-athletics
To register as a new user on FamilyID, please follow these steps.
First, create and activate your FamilyID account.
If you have not already done so, create a FamilyID account here.
After you have signed up for an account, please check your inbox for an Activation Email from FamilyID. You will need to click the link in that email to activate your account.
Once you have activated your account, you will be ready to register for our program! Please click here to watch a quick video or read a help article on how to register for the first time on FamilyID.
Click the blue Submit button. After selecting Submit, the registration will be complete. You will receive a completion email from FamilyID confirming your registration.

At any time, you may login at www.familyid.com to update your information and to check your registration(s).
