Sports Physicals

Family ID registration must be completed and all new physical paperwork must be turned in for Fall Sports as follows: by August 16, 2023 for Varsity and JV Sports and by August 23, 2023 for all modified sports.

HEALTH FORMS REQUIRED:

Please make sure you have a valid physical on file in the nurse’s office.  

Please note--Effective July 30, 2021, we are no longer be using the green Interval Health History form or the Sports Candidate Questionnaire. The information contained on these forms will now be embedded in your FamilyID registration. It is imperative that a parent or guardian complete the registration in FamilyID. Students SHOULD NOT be completing the registration.

Additionally, after consultation with our school Physician and based on the newly released recommendations from the American Academy of Pediatricians (AAP), the District is implementing a new protocol for students. Following a positive COVID-19 diagnosis, the District may now require that your child be evaluated and cleared by his or her primary care physician (PCP) and to be symptom free before returning to athletics or physical education class. Your child’s PCP will be asked to complete the Assessment/Release for Return to Play form prior to resumption of athletics or physical education. Within this form, your child’s PCP has the option to require an additional 4-Phase Graduated Return-to-Play protocol be completed prior to returning to full activity. In addition, if your child needs an inhaler or epi-pen, parents must complete the medication consent form each school year.  

Paperwork must be handed in to the nurse's office to be put on the approved health list prior to tryouts.
