    Ninth (9th) & Eleventh (11th) grade students:   

    Are mandated to have a current (within the year) physical exam.  Students may see their own physician but must bring current physical information to the health office.  If you wish, your child may have physical at school.  Watch for posted dates.

     

    Athletes: 

    Be sure to sign your student up for the specific sport they will be playing on Family ID. 

    Updated physicals can be delivered to the health office, faxed to 210-2361 or emailed to clockwood@starpointcsd.org.

    Medication:

    Any student who requires any (including over the counter - tylenol, ibuprofen etc.) medication during the school day MUST have a written physician's order (can be faxed by the office) or a medication administration form. This must be updated yearly by their physician and parent.
