• All 12th grade students need a 2nd meningitis shot in order to attend school. If you student does not have this, you will be contacted. Please have vaccine info sent to info listed below.

    MEDICATION ADMINISTRATION IN SCHOOL: If your child will be needing medication in school (over-the-counter and/or prescription), a medication order needs to be received from your child's physician. This can be achieved with an order provided by the physician or with an "Authorization for Administration of Medication" form completed by the parent/guardian and physician (located below). This is to be returned to the Health Office by the parent/guardian along with the medication in its original container. Only inhalers and Epi-pens may be carried by the student once proper consent is obtained. Until the consents are received, your child will not be able to receive or self-administer medication in school.

     

    9TH AND 11TH GRADE STUDENTS: According to New York State Health Law, students in grades 9 and 11, new entrants, and those participating in sports must have a complete physical exam performed. It is encouraged that this exam be performed by your child's primary care provider. Forms may be obtained from the Health Office or downloaded below. The physicians office may fax their own form to the number below. 

     

    12TH GRADE STUDENTSProof of 2nd dose of the Meningococcal vaccine is due within 30 days of starting 12th grade. This is mandatory and your child can be excluded from school if this is not obtained and records not provided. Please speak to your physician to guarantee your child has received all necessary vaccines for school. 

    NYS law requires each parent/guardian provide the school with an immunization (shot) record from a health care provider, health department, or official immunization record from the child's previous school that shows the child has recieved the required vaccines in order to attend school. Dates must be included in the record.
