Attendance More Info To report an absence, please call the Middle School attendance office at 210-2205 and submit a written excuse on the day the student returns to school.  The note should contain the student's name and homeroom, the date and reason for absence, and parent's signature.  Forms for required notes (pdf file - 81kb) Parents/guardians are required to submit a note when a student is absent, tardy or leaving early.
